Please Read for Important Insurance Details.Medicare Medicare requires you to be evaluated by your physician prior to your initial visit at our office and at intervals of 30-60 days while having physical therapy.
Worker’s Compensation (WC)* requires a physician's prescription and prior authorization. You should call your insurance company and verify their requirements.
Auto/Personal Injury Protection (PIP)*requires a physician’s prescription and claim number to begin physical therapy. If your PIP coverage is exhausted, you will be responsible for the outstanding balance. If your health insurance covers this amount, we will file this claim for you.
*For Auto, PIP and WC cases, it is very important that all the information we receive from you is accurate and up-to-date. You will be responsible for any financial obligation not covered by your carrier.
